Waiver of Liability I agree to release KERI MAYALL (Certified Holistic Wellness Coach, Holy Fire III Reiki Master Practitioner), of any responsibility or injury occurring from my session(s). I affirm that I am responsible for creating and implementing my own physical, mental, financial, and emotional well-being, decisions, choices, actions and results. As such, I agree that the Coach + Reiki Master Practitioner is not and will not be liable for any actions or inaction, or for any direct or indirect result of any services provided by the Coach + Reiki Master Practitioner. I understand that coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease. I understand that coaching is not to be used as a substitute for professional advice by legal, mental, medical, financial or other qualified professionals and will seek independent professional guidance for such matters. If I am currently under the care of a mental health professional, Coach will recommend that Client inform the mental health care provider.
Reiki Energy Healing methods used are designed to facilitate the client’s quest for self-improvement and relaxation. Specific additional techniques may include: Body Relaxation, Directed Meditation and Guided Imagery. I understand that Reiki is a simple, gentle, hands-on and hands-off energy technique that is used for stress reduction and relaxation. I understand that Reiki practitioners do not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. I understand that Reiki does not take the place of medical care. It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological ailment I may have. I understand that Reiki can compliment any medical or psychological care I may be receiving. I also understand that the body has the ability to heal itself and to do so, complete relaxation is often beneficial. I acknowledge that long term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself.
