WAIVER OF LIABILITY FOR PERSONAL TRAINING / GYM RENTALS (HIVE GYM)

I/We hereby understand and acknowledge that the training, programs and events held by GENESIS TRAINING LLC and THE HIVE GYM LLC may expose me to many inherent risks, including accidents, injury, illness, or even death. I/We assume all risk of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by me.

I/We hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in activity. I/We acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in. After having read this waiver and knowing these facts, and in consideration of acceptance of my participation and GENESIS TRAINING LLC and THE HIVE GYM LLC furnishing services to me, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE GENESIS TRAINING LLC and THE HIVE GYM LLC, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in the GENESIS TRAINING LLC and THE HIVE GYM training, programs and/or events.

By purchasing this training package, I/We indicate that I/We have read and understand this Waiver of Liability. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.

TERMS AND CONDITIONS

I/We agree not to hold Genesis Training LLC and THE HIVE GYM LLC or any of it's trainers liable for injuries sustained during exercise or any injuries sustained (including death) at it's facility (342 7th st, Jersey City or 392 Summit Ave, Jersey City) or any other public or private location where a training session is held. I/We agree that GENESIS TRAINING LLC and THE HIVE GYM and any of its affiliates responsible for damages of property outside of it's training facility, including but not limited to apartment gyms and apartments.

I/We also agree to a 48hr late cancellation policy where any late cancellations within 48hrs of your scheduled session or no shows will result in the forfeit of the session and cannot be rescheduled.

I/We agree once a training package has started, the remaining sessions are non-refundable for any reason, but sessions may be transferred to another client.

I/We understand that if enrolled into the personal training services we will have access to GENESIS TRAINING LLC's private training app and other resources such as nutrition guidance and videos, but it is my/our responsibility to utilize them to optimize my/our results.
