Prior to your appointment:
1. You must cancel your appointment if you have been exposed to or tested positive for COVID-19 within the last 14 days
2. Complete and email your candidate information sheet
3. Complete your financial disclosure statement and email the confirmation number

4. Establish an authorized candidate campaign committee or have one in good standing with the State Board (Statement of Organization)
