Please fill out the form sent to your email within 24 hours of your appointment. All appointments except "Follow Up" appointments are $200. If all documents and forms required are present at that appointment, we will waive that fee and the appointment will be free. If appointment is cancelled or rescheduled within 24 hours or documents and forms are not present, the fee will not be waived.
