6. Fill out personal information form.

If your appointment is for a vaccine, please bring ALL insurance cards with you to your appointment. Covid-19 vaccines are free to the patient but we will bill the administration of the vaccine to insurance. This includes Traditional Medicare, Indiana Medicaid or commercial insurance drug plans.
