*GIFT CARD AND PACKAGE PURCHASE AGREEMENT*

This agreement is made and entered into on date of purchase by and between Opulence Beauty, LLC ("Spa") and purchaser for the purchase of a gift card or package.

*1. PAYMENT TERMS*

The client agrees to pay the full amount due for the gift card or package at the time of purchase. The client understands that the gift card or package is non-refundable.

*2. NON-REFUNDABLE*

The client acknowledges and agrees that all gift card and package sales are final and non-refundable. No refunds or credits will be issued for any reason.

*3. TERMS AND CONDITIONS*

The client agrees to abide by the spa's terms and conditions, including but not limited to:

- Cancellation policies: appointments must be cancelled before 48 hours of appointment time otherwise full service price is due or gift card/package amount will be forfeited

- Late arrival policies: arriving 10/15 min late for your scheduled appointment you will be asked to reschedule your appointment and charged the same day cancellation fee (100% of service price). On a case to case basis: we will take you back after the 15 min allowing it to cut into your service time and the session will be cut short with the remaining time you have scheduled.

- Service guidelines: client agrees to remaining professional with every technician with Opulence Beauty and agrees to speak up if there is something in the session you are not comfortable with and be able to communicate that to the technicians.

*4. GIFT CARD TERMS*

Gift Cards are redeemable for services and products at Opulence Beauty ONLY. Gift Cards are NOT redeemable for cash or credit.

*5. PACKAGE TERMS*

Packages are non-transferable and must be used by the Client within the specified timeframe.

*6. RELEASE AND WAIVER*

The Client releases and waives any claims or liabilities against Opulence Beauty, its officers, employees, and agents.

*7. GOVERNING LAW*

This Agreement shall be governed by and construed in accordance with the laws of North Carolina.

*8. ENTIRE AGREEMENT*

This Agreement constitutes the entire agreement between the parties and supersedes all prior or contemporaneous agreements or understandings.

By signing below, the Client acknowledges that they have read, understand, and agree to the terms and conditions of this Agreement.
