Public Benefit Application Assistance

Our team assists neighbors in completing and submitting Public Benefit Applications (virtually) in both Illinois and Missouri.

Please fill out the form below for an appointment with a member of our Community Resource team. During this appointment our team will call you at the number you provide and assist you in completing a Public Benefit Application.

This is not an interview with the State, the St. Louis Area Foodbank is a 3rd party community partner and assists with application submission.

Note: Only 1 appointment is necessary for each household.

The following information for each household member will be helpful for your appointment:

  • Date of Birth
  • Social Security Number
  • Utility Bills
  • Housing Information (Rent/Mortgage/Tax Bill)
  • Proof of Income (For everyone in household for last 30 days)
  • Child Day Care Expenses (if applicable)
  • Medical Expenses
