I acknowledge that Relax Therapeutic Massage & Integrative Bodyworks WILL BAN ANYONE from the privilege of rescheduling online and/or purchasing gift certificates or Loyalty Packages online if you have abused the rescheduling option, i.e. rescheduling using the same gift certificate or Loyalty certificate more than 3 times. After the 3rd time rescheduling you either keep your appointment or forfeit the certificate. IT WILL NOT BE REFUNDED!
