2) THIS PACKAGE IS VALID FOR THE TIME PERIOD STATED WHEN PURCHASING THE PACKAGE
3) THIS PACKAGE IS VALID FOR THE TOTAL SUM (£) STATED WHEN PURCHASING THE PACKAGE
4) CLASS CANCELLATION POLICY: 12 HOURS

I understand the various risks associated with an exercise programme & it is my desire to participate. I have not withheld any relevant information regarding my physical condition, which may affect me during or following this session. I agree that Emily Cohen & Roo Hamer are not responsible for any injuries sustained by me during my exercise session. I hereby release Emily Cohen, Roo Hamer and all parties involved from any responsibility. I have read the terms & conditions & understand the rules, regulations and policies (overleaf).

I understand that it is my responsibility to inform both Emily Cohen & Roo Hamer of my fitness condition/level and any physical injuries, health concerns (previous and current), pregnancies and medications taken should I believe this is necessary. I agree that both Emily Cohen & Roo Hamer are not responsible for any of the above.

I agree to adhere to the 12 hour cancellation policy and will pay for my late cancellations.
