HOW IT WORKS
- Review available time slots to schedule an appointment for your 1st, 2nd, 3rd, or Booster COVID-19 Vaccine. (Recipients must be at least 5yrs old if scheduling an appointment for a Pfizer vaccine and at least 18yrs old if scheduling an appointment for Moderna or J&J. If you're under age 18, you must be accompanied by a parent or guardian.)
- Please arrive at your scheduled appointment time with your government issued ID and a copy of your Medicare card, Prescription card, or Health Insurance card. There is no cost to you for the vaccine!
- If this is your 2nd dose or booster dose, please remember to bring your COVID vaccine card.
- Please only come at the time of your appointment and do not come early as space is limited.
- If you received your 1st dose, we will automatically schedule your 2nd dose appointment assuming you rec'd either Pfizer or Moderna.
- Once you have booked your appointment for your 1st shot, please complete our Vaccine Consent form prior to arriving for your 1st shot. (Link Below)
- After you have scheduled an appointment, you should receive a text message and email confirming your appointment. If you did not receive a text message, either the cell phone number you entered was entered incorrectly or you entered a landline phone number. If you did not receive an email, please check your email spam folder. If you do not see an email there, it’s possible you entered your email address incorrectly. Feel free to call us and we can verify your email and resend your confirmation.
- Depending on how soon your appointment is to the day/time you booked it, you will also receive a text message reminder 24 hours prior to your scheduled appointment.
