- You may cancel or change your appointment time up to 24 hours before your appointment starts (Vaccines are time sensitive)

- Must work or be an Idaho resident to receive a vaccine in Idaho

-Must work or be an Oregon resident to receive the vaccine in Oregon

-Must be 16 years or older to receive the Pfizer vaccine

-Must be 18 years or older to receive the Moderna & Johnson and Johnson vaccine

- A second dose appointment will be scheduled at the time of 1st dose appointment

-In order to be guaranteed a second dose, your appointment must be scheduled exactly 3 weeks after the 1st dose with the Pfizer vaccine and exactly 4 weeks with the Moderna vaccine (If this does not fit in your schedule, we cannot guarantee the 2nd dose; however, we will do our best to accommodate scheduling of your second appointment)
