East End Surf Club Release and Liability Waiver

All Students, and the guardians of participating students of East End Surf Club, LLC (herein and after in this document as EESC) must read and complete the following legal form before participating or enrolling in any EESC activity.

I hereby agree for myself on behalf of my minors, heirs, personal relatives, next of kin, and whomever else may have interest, hereby release, waiver, and hold harmless EESC and their officers, instructors, agents, and/or employees from liability from any and all claims for any and all injury, disability, or death, or loss or damage to person or property whether arising from the negligence of EESC or otherwise, sustained as a result of any EESC activities to the fullest extent permitted by law.

I acknowledge and agree that water sports and surfing are inherently risky and potentially dangerous athletic activities in an uncontrolled natural environment. Hazards including, but are not limited to; rip currents, waves, rocks, sand bars, reef, jetties, sea creatures, surfboards, and other people in the water and on the beach. Participating in EESC activities involves inherent risks regardless of supervision or lack of supervision by EESC instructors.

I knowingly and freely assume all risks related to EESC activities and accept full responsibility for my participation or the participation of any minor on whose behalf I am a parent or legal guardian in signing this document.

I verify that I am a (or any minors on whose behalf I am a parent or legal guardian of) competent swimmer, mentally sound, in good physical health, and fully capable of participating in any and all strenuous athletic activities related to surfing. I am not under the influence of drugs, or alcohol, or any other substance that may impair my ability to swim or follow EESC instructions and rules.

Permission to obtain medical assistance: I permit EESC to perform minor medical first aid or, in the event of an emergency, get medical assistance for myself (or any minors on whose behalf I am a parent or legal guardian). The expenses for any such medical emergency shall be my sole responsibility and not that of EESC.

Promotional use of images: I agree and understand that during activities, videotape and/or photographic records can and will be maintained and that EESC will have the right to use video or photographic records of activities for its own promotional and/or commercial purposes without monetary compensation.

Cancellation Policy

We have a 24-hour cancellation policy, so please call or text your instructor 24 hours in advance if you need to cancel or reschedule. We will cancel lessons for conditions not suitable for surf lessons, including rip currents, big/rough surf, strong winds, thunder and lightning, heavy rain, and a flat ocean. We do not cancel lessons for light rain or clouds.
