If you need additional cards, please let the front desk know when you arrive.
Do not fill out anything on the cards
We do not handle any federal fingerprints to include teachers, nurses, federal contractors. If you need federal fingerprints and check, you will need to go to a GAPS location.

This appointment is for someone that has or has previously had a manager's permit for the City of Woodstock. You will not have to be re-fingerprinted.
