Our team assists neighbors in completing and submitting Public Benefit Applications (virtually) in both Illinois and Missouri. This includes both SNAP (Food Stamps) & Medicaid.
Please fill out the form below for an appointment with a member of our Community Resource team. During this appointment our team will call you at the number you provide and assist you in completing a Public Benefit Application. This includes both SNAP (Food Stamps) and Medicaid.
This is not an interview with the State, the St. Louis Area Foodbank is a 3rd party community partner and assists with application submission.
Note: Only 1 appointment is necessary for each household.
The following information for each household member will be helpful for your appointment:
- Date of Birth
- Social Security Number
- Utility Bills
- Housing Information (Rent/Mortgage/Tax Bill)
- Proof of Income (For everyone in household for last 30 days)
- Child Day Care Expenses (if applicable)
- Medical Expenses
