OUR POLICIES:

APPOINTMENTS AND CANCELLATIONS

· If you fail to keep your scheduled appointment or neglect to cancel the appointment with at least 48 hours’ notice (unless there is an emergency), you will be charged a fee of $100. This is necessary because a time commitment is made to you and is held exclusively for you.

· If you do not show up for a scheduled appointment or cancel with less than 48 hours’ notice on 2 occasions within the past year, you will be discharged from the practice.

· If you are late for a visit, you may lose some of the time reserved for you. Your scheduled treatment may not be able to be completed and you may be charged a late cancellation fee of $100 if you are more than 5 minutes late.

Medical Weight Loss Follow Up VIRTUAL VISIT

30 minutes @ $50.00

This is a visit with our nurse to check-in on your experience with the prescribed medication.
Location: Simple Practice telehealth link will be sent to you day of your visit

CONTRAVE: Must be in person follow up to check blood pressure and receive medication to take home.

SEMAGLUTIDE: May be in person or virtual (choose the other visit type).

Credit card required to reserve your spot. In the case of late cancellation or no show, your credit card will be charged $50.
