Information noted below should be for the person who is getting tested. Each family member will need to be entered separately.

Please note there are NO REFUNDS for cancellations (less than 3 hours ahead) or missed appointments. If you are more than 10 minutes late, that is considered a missed appointment.

Group testing or Onsite testing: Please call 202-775-8500 for inquires.

JAPAN FORM: Please fill out the top portion and present this form PRIOR to your test. This form will be emailed to you Monday - Friday after your test results are returned. This will not be emailed on the weekends. Do not select our Expedited test on Friday if you have a Japan Form as you will not get the form until the following business day.

Farragut Medical and Travel Care does not take insurance directly, but we will give you the necessary documentation to seek reimbursement from your insurance provider. Please reach out to them for specifics on what is their reimbursement policy or specific requirements.

MEDICARE:
We do not take MEDICARE and you are not able to file with MEDICARE. Since we do not participate with MEDICARE, that means that MEDICARE will not accept nor pay for any claims sent to them from a member. If you have secondary insurance or supplemental insurance to MEDICARE, they will also not accept nor pay for any services due to the non-participation with MEDICARE by our Providers.
