CANCELLATIONS/RESCHEDULE - If you would like to reschedule your appointment, please TEXT the desired date and time for your new appointment request to 513-278-7668, we will send confirmation of your new appointment
-Appointments canceled less than 24 hours before time of service will not be rescheduled
AESTHETICS – All Aesthetic appointments including Skin Resurfacing with OPUS, Botox, Fillers require an initial consultation to determine the appropriate treatment plan. The consultation fee is $50 and includes a detailed history and one-on-one evaluation with the Nurse Practitioner
IV HYDRATION - All IV Hydration Treatments require baseline hydration labs prior to your initial infusion for an additional cost of $65
-This is a one-time upfront fee. Repeat labs may be necessary every 90 days
VAGINAL STEAMS – All initial vaginal steam appointments require a vaginal steam consultation to determine the appropriate herb formulation. The consultation fee is $19 and includes a detailed menstrual and Women's Health History.
BODY CONTOURING/LIQUID LIPO – All initial body contouring and/or Liquid Lipo appointments require an initial consultation to determine appropriate treatment plan. The consultation fee is $50 and include a history and physical
FORMS OF PAYMENT – We accept Cash, Credit Care, Care Credit, Affirm (through our Rewards App) and FSA/HSA
-It is the responsibility of the client to know which services or products are covered under FSA/HSA
REFUNDS – UNDER NO CIRCUMSTANCES WILL A REFUND BE ALLOWABLE FOR ANY SERVICE PROVIDED OR PURCHASED AT UZIMA, INCLUDING BUT NOT LIMITED TO FSA OR HSA AS FORMS OF PAYMENT!
ARRIVALS – We suggest that you arrive 15 minutes before your scheduled appointment time if you are a first-time client in order to complete the necessary client forms
LATE ARRIVALS – We regret that late arrivals (15 minutes) will not receive an extension of scheduled service times and will lose their non-refundable deposit
VALUABLES – We suggest that you leave valuables at home. Uzima Rejuvenation is not responsible for any items that are lost or stolen.
CELL PHONES – Please mute cell phones. Cell phone use is not permitted during a treatment or service. Please direct any questions or concerns to staff.
