Thank you for choosing Pure Diagnostic. To make the process easier and faster
please make sure that you:
1) Print and fill out the forms attached to this email for each individual that is testing before coming in for your appointment.
2) Write with legible handwriting to avoid any mistakes. Additionally, please do not forget to write your email in order to receive the results via email.
3) Bring in an identification card as well as a PHYSICAL insurance card for each individual that is coming in for testing. Our self pay cost is $100 and we accept card or cash:)
