Welcome!
Your appointment will be in person and approximately one hour.
Please print and fill out the New Patient Intake Form prior to your appointment. You are welcome to wear a mask but no longer required. Please arrive on time, not early or late, to ensure you do not cross paths with other patients. Please bring your own water. Wear loose fitting warm clothing so that nothing needs to be removed for your treatment. Feel free to bring your own small blanket from home if you'd like.
Your initial session is $200. Follow ups are $150. You're welcome to pay with cash, credit card, HSA card, or Venmo (@fivepinsproject). We offer a $10 discount for those who pay with cash.
We can provide a receipt or superbill if you need one for insurance purposes.

***Cancellation policy***
Cancellations made less than 24 hours before the treatment time are subject to a late cancellation fee. The fee is the full price of your treatment.
If you are 15 minutes late for your appointment I will not be able to see you and it will be considered a late cancellation.
FOR SLIDING SCALE PATIENTS: The late cancellation/no-show fee is the full price of a regular rate treatment ($150).
