Our team assists neighbors in completing and submitting SNAP Applications.

Please fill out the form below for an appointment with a member of our SNAP Outreach team. During this appointment, our team will call you at the number you provide and assist you in completing a SNAP Benefits Application. SNAP is the Supplemental Nutrition Assistance Program ( formerly known as Food Stamps). If eligible, this program can help your family access additional food.

Note: Only 1 appointment is necessary for each household

The following information for each household member will be helpful for your appointment.

• Date of Birth
• Social Security Number
• Utility Bills
• Housing Information (Rent/Mortgage/Tax Bill)
• Proof of Income (For everyone in household for the last 30 days)
• Child Day Care Expenses ( if applicable)
• Medical Expenses (ONLY if you are 60 and older and/or disabled)
