Vaccines are subject to availability. Insurance (medical and or prescription) plans are accepted on most vaccines. COVID-19 Vaccines are offered Free of Charge.

In order to receive select vaccines in the state of Indiana, you must meet certain eligibility criteria set by the state. Please review the current criteria carefully to confirm that you are eligible. If you make an appointment to receive a vaccine, and we determine that you are or may not be eligible, we may have to cancel or adjust your appointment. By continuing to make an appointment, you agree you meet the eligibility qualifications and consent to be contacted via phone, text, and or email in the case of any cancelations and or adjustments.
