A NON-REFUNDABLE retainer will be charged to your saved card on file within 24 hours of booking appointment. By continuing forward with booking an appointment, you authorize Angelina M. Oliver /( A.M.O. Aesthetics) consent/approval to charge your saved card on file for the NON-REFUNDABLE BOOKING RETAINER and cancellation charges.
Fees are as follows:
Non-Refundable Retainer:
Fibroblast: $100
Plasma Sculpt: $50
Cancellation Fee: For Fibroblast, $100 within 24-48 hours of scheduled. For Plasma Sculpt, $50 within 24-48 hours of scheduled.

PLEASE NOTE: You forfeit your NON-REFUNDABLE BOOKING RETAINER if you cancel your scheduled appointment AND DO NOT RESCHEDULE IMMEDIATELY ON THE DAY YOU CANCEL YOUR APPOINTMENT. Also, if you cancel within 24-48 hours of your appointment date, you will be required to pay another NON-REFUNDABLE BOOKING RETAINER if you should book for a new date. Due to time and preparation for this treatment process, changing your appointment date leading up to your scheduled appointment, as well as within 24-48 hours of your scheduled appointment, will result in a day of lost wages for the service provider, due to this service cannot be rebooked with another client within a short period of time, or within 24-48 hours, resulting in a day of wages lost. Therefore, the NON-REFUNDABLE BOOKING RETAINER is applied towards the $100 cancellation fee. There are no exceptions for rescheduling reasons unless A MEDICAL EMERGENCY (DOCUMENTED).
If you reschedule 2 times in a row within the 24-48 hour window of your scheduled appointment, you will be charged a $200 cancellation fee. This fee will not be applied to future services.
Anyone more than 30 minutes late without proper notification to Angelina M. Oliver, is automatically canceled. Upon cancellation, your booking retainer is forfeited and may not be counted towards your future treatment. You will be charged a $150 cancellation fee for the day. If you do not want to be canceled, you must contact Angelina M. Oliver directly at 304-896-9573 prior to the 30 minute time limit of your scheduled time and let her know of your tardiness for her to confirm that she can still perform the treatment) In order for you to rebook, you will be required to pay another booking retainer to re-book. No exceptions.
If you are NO CALL, NO SHOW (meaning you do not contact Angelina M. Oliver to cancel and you miss your appointment), YOU WILL BE CHARGED: FOR FIBROBLAST A $300 CANCELLATION FEE; FOR PLASMA SCULPT, $150 THIS FEE WILL NOT GO TOWARDS A FUTURE SERVICE.
If you reschedule and miss the 3rd appointment, YOU WILL BE CHARGED 50% OF THE TREATMENT COST OF THE TREATMENT YOU SCHEDULED.
By moving forward with booking an appointment for treatment, you give Angelina M. Oliver/ A.M.O. Aesthetics consent and authorize Angelina M. Oliver/A.M.O. Aesthetics to charge the credit card on file for nonrefundable booking retainer, as well as the cancellation fees per policy. You acknowledge that you are responsible for 50% of the treatment cost. Therefore, you are responsible for 50% of the full balance of scheduled appointment and will be charged to the credit card on file. You agree to these terms and agree to pay 50% of the wages lost due to missing your appointment. You agree that you are responsible for this payment and to follow through with payment.

Please notify Angelina prior to booking if you are allergic to Benzocaine, Tetracaine or Lidocaine.
You are NOT a candidate for the Fibroblast Treatment if you:
- Are pregnant or nursing. Please wait 6-9 months postpartum/nursing to book.
- Keloid scar (yes, even just one!) No exceptions.
- Use Accutane (must be off at least 12 months, no exceptions)
- Active cancer or chemotherapy (must be finished with chemo 6 months before booking and provide a doctors note)
- Active acne or breakouts throughout the area for treatment.
- On Blood Thinners (including aspirin)
- On Steroids
- Psoriasis - Active Eczema on face or treatment area. PLEASE NOTE that this treatment activates your body's inflammatory response, which can cause a flare up to the treatment area, which is why it is contraindicated.

- Had a microneedling treatment.
- Have a tan to the area for treatment.
- Had a skin laser treatment to the area for treatment.

- Used any skin products containing Alpha Hydroxy Acids: Glycolic acid, Lactic acid, Malic acid, Tartaric acid, Citric acid, Mandelic acid.
- Chemical peel within 6 weeks of treatment.
STOP USE OF THESE TREATMENTS/PRODUCTS 30 DAYS BEFORE SCHEDULING TREATMENT.
PLEASE WAIT TO SCHEDULE IF WITHIN 2 WEEKS OF APPOINTMENT TIME YOU HAVE:
- Using skincare products that contain Retinol, Retin-A or Tretenoin. STOP THESE PRODUCTS AT LEAST 2 WEEKS prior to your appointment for treatment.
-Have been on antibiotics in the last week. Must wait at least 10 days from the last dose prior to scheduling an appointment for treatment.
- Used any over-the-counter skin products containing benzoyl peroxide, salicylic acid - Products that exfoliate the skin. STOP USE OF THESE TREATMENTS/PRODUCTS 2 WEEKS BEFORE SCHEDULING TREATMENT.
PLEASE WAIT TO SCHEDULE IF THE FOLLOWING 2 WEEKS OF APPOINTMENT DATE YOU-
- Have a special event / family photos (healing skin = flaky crusts on the skin from 5-7 days for facial treatments, to 10-14 days for body treatments)
- Are going on a vacation
- Have a 5k / Marathon / anything that involves sweating. NO sweating during the initial or first phase of the healing process, when crusts are on the skin.
- Cannot avoid direct water / ex. being in the pool, lake trips, etc. common activities during summer.- Going on a vacation
- CANNOT AVOID THE DIRECT SUN TO THE AREA FOR TREATMENT. SKIN MUST NOT BE TAN AND OF NATURAL COLOR BEFORE IT CAN BE TREATED. IF YOUR SKIN IS TAN, YOU MUST WAIT AT LEAST 30 DAYS BEFORE SCHEDULING A TREATMENT TO ALLOW THE SKIN TO GET BACK TO IT'S NATURAL COLOR. A TAN REPRESENTS EXTRA MELANIN IN THE SKIN THAT CAN CAUSE ADVERSE EFFECTS AND DAMAGE YOUR RESULTS.

The specialist's skills, technique, and performing your treatment is 49% of your results, but the rest is up to YOU. I agree that if I am Diabetic, I am not a candidate for treatment unless my primary care provider approves me for treatment to determine that my Diabetes is well controlled, being managed with daily medication and my levels are within normal range. I will not move forward with scheduling an appointment for treatment prior to discussing with Angelina M. Oliver, RN/A.M.O. Aesthetics, my Specialist. PRE, DURING AND POST APPOINTMENT INFORMATION
PLEASE NOTE PRIOR TO YOUR APPOINTMENT: Aloe Vera Gel with Lidocaine and Witch hazel toner can be purchased in preparation for your treatment. These products can be purchased at any Walmart or Pharmacy and is recommended to help keep you comfortable during the healing and aftercare process.
DAY OF APPOINTMENT:
No Caffeine
No Ibuprofen
No Exercise
2 Days prior to appt: Avoid taking Ibuprofen as it can cause sensitivity of the skin, making the numbing agents used for the treatment less effective. Also, no alcohol.
2 weeks prior to appt:
No Botox. No Retinol or Retin-A, Tretenoin. No over-the-counter skin products for exfoliation or acne, containing salicylic acid or benzoyl peroxide.
No tanning beds or tanning sprays or creams (if you are sunburned, please reschedule for at least 30 days out from your last day tanning)
No antibiotics- If on an antibiotic, please book 10 days post last pill
4 weeks prior to appt:
No chemical peels, microdermabrasion, microneedling, laser treatments. No tanning beds or tanning sprays or creams. No products to the skin containing alpha hydroxy acids to the treatment area. No products containing Vitamin C.
DURING APPOINTMENT TIME
No guests are allowed. (Due to Covid)
No children are allowed. No exceptions.
POST CARE - FOLLOWING APPOINTMENT:
Keep skin dry for the first 3 days. USE WATER ONLY or Witch Hazel Toner to cleanse the area, using a cotton round to DAB or PAT IT ON, then PAT IT DRY WITH THE COTTON ROUND. NO RUBBING. PAT DRY ONLY. No Sweating
No Direct Sunlight
No Direct Water to face, no showers, BIRD BATH ONLY. Keep the area of treatment dry. USE AFTERCARE PRODUCTS PROVIDED FOR YOU AS INSTRUCTED. You can wear mineral makeup AFTER CRUSTS HAVE FALLEN OFF ON THEIR OWN. DO NOT REMOVE SCABS/CRUSTS PREMATURELY. THE CRUSTS ARE NEEDED FOR THE HEALING PROCESS AND FOR ACHIEVING BEST RESULTS. Scabs/crusts will fall off from post day 3 to day 7-10 on average. You can use beta glucan and hyaluronic acid serums, and a MINERAL ZINC OXIDE SUNSCREEN SPF50+ to protect from UV Rays during the healing process. Can use RED LED Light therapy for faster healing and for hyperpigmentation issues. Do not use Vitamin C Serums to the treatment area until at least 30 days after treatment. Final healing/skin tightening and rejuvenation of the skin is not complete until 12 weeks after treatment to see the final results.
FOR TREATING EYE AREA:
Swelling around the eyes is expected for post day 1-3. Apply ice packs (wrapped in cloth or paper towel to protect the skin) to the eye area for 20 minutes, 2 to 4 times daily to decrease swelling. Swelling will be worse in the morning (from laying down overnight). Physical activity during the day will help the lymphatic system to decrease swelling of the treatment area. If you have a LED Light Therapy facial mask, the red light therapy can be used to help promote collagen production, revitalize skin cells in the healing process, and improve blood circulation to the treated area. The mask can be used after applying serums to the skin (wear for about 20 minutes). If your LED Light facial mask has the Green LED Light mode, the green light can also be used to reduce hyperpigmentation and promote lymphatic drainage to help decrease swelling.
