Before booking, please be sure you did your due diligence to learn about either Plasma Fibroblast or Jet Plasma. I encourage everyone to either get a consultation or get in touch with me if you have any hesitations or questions about your candidacy for plasma fibroblast. I may ask to see photos, do a patch test and/ or suggest products to aid in prepping your skin for fibroblast. If you are Fitzpatrick skin type 4- 5, I highly recommend getting a patch test first or getting in touch with me before booking. And unfortunately fibroblast is not recommended for anyone with FP skin type 6 because of the high risk of hypopigmentation/ hyperpigmentation. Fortunately, everyone is a candidate for Jet Plasma. Jet plasma cannot be administered on pregnant women, anyone with a pacemaker, anyone who is hypersensitive, anyone with cancerous lesions, anyone with an implanted neurostimulator, or any kind of implanted slow release medication device.
After choosing the service of your choice you will then select your appointment day/ time. I am accepting appointments on Thursdays and Saturdays. After selecting your appointment time, there is a short intake form which will help me understand your basic health history and candidacy for treatment. Payment will be taken on the final page where I accept PayPal or credit card towards your 25% deposit, unless you prefer to pay in full. Balances are due on the day of your appointment.
Cancellation policy: More than a week in advance cancellations, your deposit will be refunded. If you reschedule more than a week in advance, we will push forward your deposit to hold your new time. If you cancel within a week, the deposit is not
refunded. Only limited exceptions will be made. Please contact me as soon as you know you will need to cancel or reschedule your appointment. If you reschedule within a week, I will keep your
deposit and require another 25% deposit to hold your new appointment. Appointments cancelled within 72- 48 hours will not receive a
refund of their deposit and I will charge an additional 20% fee, based
on your service price, not put towards a new appointment. If
you reschedule within 72- 48 hours, 25% deposit is not refunded and I charge a new 25% deposit to hold your
new appointment, plus a 20% fee. If an appointment is cancelled with 24 hours or less, or you are a no-show,
I keep your deposit and charge you the full price of the service at the
quoted price. In order to re-book your appointment, I will require a second full payment at quoted price, in addition
to any policy fees deemed appropriate. Your 25% deposit is only refundable if appointment is not cancelled or rescheduled within a week before your scheduled appointment. Thank you for honoring and respecting my policies. <3

As soon as you book, you will receive a confirmation email with the appropriate Waiver Form that needs to be completed at your earliest convenience. It contains a lot of useful information that you will need to read through in preparation for your treatment. Both are linked above. I will keep this waiver/ questionnaire safe in your file.
