Motivity Chiropractic

We require a $50 deposit to schedule a new patient appointment. This deposit will be applied to the balance of your new patient appointment. Should your insurance cover all charges, you will be refunded the deposit at your new patient appointment. If you cancel, miss, or reschedule within 24 hours of the scheduled appointment, your deposit will not be refunded.

Once you schedule your new patient appointment, please complete and submit the new patient paperwork to info@motivitychiropractic.com.
