Membership Cancellation Policy

By subscribing, you agree to a recurring payment at the beginning of each month for 12 months unless the membership is canceled. If you choose to cancel your membership, please note that re-subscribing will not be available until one full year has passed from the cancellation date.

This policy ensures consistency and fairness for all members. If you have any questions, please don’t hesitate to reach out. Thank you for your understanding!

Late Arrival Policy

All scheduled appointments will end at the scheduled ending time in order for us to stay on schedule. Clients who arrive late to their scheduled appointment will be charged for the full session and will not receive a time extension. For your first appointment, please arrive 5 minutes prior to your scheduled appointment time to allow time to complete the Client Intake Form. For all other appointments, please arrive 5 minutes before your scheduled appointment time to allow time to undress and get on the table to enjoy a relaxed and unhurried experience.

Chaperone Policy

A chaperone of the client's choosing may attend these sessions if the client wishes. If the client is comfortable with attending sessions alone below is a waiver of the chaperone policy. This will act as a continuous waiver, if the client at any point wishes to have a chaperone attend they must submit a written notice to Carve Body Work of their revoked wavier.

COVID-19 Screening and Liability Release

The health and safety of our clients and therapists is our highest priority. Therefore we are taking every precaution to prevent the spread and reduce the risk of exposure to infectious diseases including COVID-19. These precautions include elevated sanitation practices and routine screening for symptoms of COVID-19.

I also acknowledge that close contact with people increases the risk of infection. Therefore I understand that this business and massage therapist cannot be held liable for any exposure to the COVID-19 virus or any other contagion. I therefore release the business and massage therapist from all liability for any harm that may result from exposure.

Draping Policy

Clients will be appropriately draped with a sheet and/or towel at all times during their massage. Only areas of the body that are currently being treated will be exposed. The breast and genital areas will always remain draped and are never massaged.

Liability Release

I understand that the massage service offered is for the therapeutic purpose of general wellness, stress
reduction, and relief of muscular tension.

Information about massage therapy, potential benefits, effects, risks, contraindications, and possible alternative therapies have been explained to me and I understand this information. I understand the risks associated with massage therapy include, but are not limited to:

- Superficial bruising

- Short-term muscle soreness

- Exacerbation of undiscovered injury

I have been given the opportunity to ask questions about massage therapy and my questions have been answered to my satisfaction.

If I experience any pain or discomfort, I will immediately inform my therapist so that the pressure or techniques can be adjusted to my comfort level.

I will not hold my massage therapist responsible for any pain or discomfort I experience during or after the session.

I have provided my therapist with an accurate and complete medical history and agree to inform my therapist of any new diagnoses, or changes in my health or medications.

I do not have any injuries or conditions that prevent me from receiving massage therapy.

I understand the importance of informing my massage therapist of all medical conditions and medications that I am taking, and that there may be additional risks based on my physical condition.

I understand that I or the massage therapist may terminate the session at any time.

I understand that massage therapy is not a substitute for a medical examination or treatment, and that I should see a physician or other qualified health specialist for any mental or physical ailment of which I am aware.

I understand that massage therapists do not diagnose illness or disease, and nothing said during the treatment should be construed as such. My consent is informed and voluntary and I understand that I may withdraw my consent at any time except for actions already taken.

I release the massage therapist and business from all liability for any harm that may unintentionally result from this treatment.

By purchasing this membership I agree to the conditions as outlined above, and I release the massage therapist and business from all liability for any harm that may unintentionally result from this treatment.

Massage Termination

Only professional massage and bodywork services for relaxation or therapeutic purposes are offered at this massage practice. Massage services will be terminated immediately in the event ofinappropriate conduct of any kind. This includes harassment, threatening speech or behavior, sexual advances or requests, or disrespectful actions or language. A session will not be conducted if the client is under the influence of drugs or alcohol. If the massage is terminated for any of these reasons, full payment for the scheduled session is still required.

Privacy Policy

All communication between Care Body Work practitioners and clients are considered protected health information and will be kept confidential. This information will not be released unless a written and signed consent form is submitted or a court order is filed. Some treatment may include photographs, this will also be considered confidential protected health information and they will not be shared without a signed release or court order. Agreement to the policy is not a formal agreement to photography and clients may opt out at their own discretion during the intake evaluation process.
