Note:

1. Book the day and time
2. Sessions are weekly
3. If you want to schedule more than one click recurring
4. Fill out INTAKE FORM and fill out all required fields
5. Fill out RELEASE OF LIABILITY AND CONSENT FORM
6. All fields must be answered to submit
7. Mark not applicable in a given form if it does not apply
8. If you are paying by cash, check or venmo, on the payment page there is a Package, gift, or coupon code write 'cash', 'check', or 'venmo' and press apply (you will see a green check and the complete appointment-confirm button appear.
9. For all others click Pay Now- Pay & Confirm, once you complete payment, you will receive an email with your receipt and a confirmation of your appointment.
10. Make sure you complete all steps whether paying in cash or paying now. If you do not immediately receive an email, you are not scheduled.
